Thrombectomy Is Safe, Feasible Tx for Primary Isolated ACA Occlusions

Background: Endovascular stroke treatment with mechanical thrombectomy (MT) has shown a possible therapeutic benefit over best medical treatment (BMT) for distal medium vessel occlusions (DMVOs) of the middle and posterior cerebral arteries but there are little data for the treatment of the anterior cerebral artery (ACA). Primary isolated ACA occlusions are not common, making up Objective: To evaluate clinical and safety outcomes between thrombectomy and BMT with or without IV thrombolysis for primary isolated ACA DMVOs. Design: Case-control study of the TOPMOST registry analyzing data collected during almost a 9-year study period. Participants/Methods: Patients treated with thrombectomy or BMT alone for primary ACA DMVO distal to the A1 segment were analyzed and compared. Early outcome was measured by improvement of National Institutes of Health Stroke Scale (NIHSS) scores at 24 hours. Favorable functional outcome was established as a modified Rankin scale scores of 0 to 2 at 90 day more...
