What Should Be the Standard Adjuvant Tx for Stage III UC?


Background: Patients with stage III endometrial cancer (UC) need more than surgery. PORTEC trials showed chemoradiation (C-RT) was better than adjuvant radiation alone, particularly for women with serous or stage III UC. Objective: To compare the efficacy and safety of adjuvant chemotherapy (CT) versus adjuvant C-RT for advanced UC. Design: Randomized multicenter trial run by NRG (National Surgical Adjuvant Breast and Bowel Project, Radiation Therapy Oncology Group, and Gynecologic Oncology Group). Participants: Women with stage III or IV UC of any histology, women with stages I or II clear cell or serous UC as well as patients with a positive peritoneal cytology. Methods: Patients were randomized 1:1 between C-RT and CT alone. All patients had hysterectomy and bilateral salpingo-oophorectomy, with pelvic and para-aortic dissections/biopsies optional.
